About TAMS - AC

TAMS-AC is the leading provider of a comprehensive range of culturally appropriate primary healthcare services to enhance the health and emotional well-being of the Aboriginal and non-Aboriginal community of Tamworth and surrounding districts. TAMS-AC is an Aboriginal community-controlled health service on the land of the Gomeroi people, based in Tamworth NSW. TAMS-AC has successfully delivered culturally appropriate primary healthcare services for First Nations People for close to 30 years. In addition to primary healthcare and day to day health services provided by a team of onsite doctors and nurses, TAMS-AC coordinates access to a range of visiting specialist services and community programs.

About the Role

The Case Worker Specialist Homelessness Service provides client centred case management support to homeless people and those at risk across the Tamworth, Gunnedah and Liverpool Plains (LGAs). This includes working with young people, single men, single women and families within the listed (LGAs) and more remote communities within the district. The program operates within the DCJ Specialist Homelessness Services framework. The Case Worker will apply the Tamworth Aboriginal Medical Service- Aboriginal Corporation's vision across all work practices and will contribute to a cohesive and productive team and organisation.

Capability Requirements

  • Certificate IV in Community Services - Case Management (or equivalent or higher) or working towards the same
  • Must hold a valid Working With Children Check
  • Must hold a valid National Police Check
  • Current drivers’ licence
  • Proven experience working with, and/or deep understanding of issues impacting homeless people and those at risk
  • Experience working in an Aboriginal or primary health care setting
