TAMS-AC is the leading provider of a comprehensive range of culturally appropriate primary healthcare services to enhance the health and emotional well-being of the Aboriginal and non-Aboriginal community of Tamworth and surrounding districts. TAMS-AC is an Aboriginal community-controlled health service on the land of the Gomeroi people, based in Tamworth NSW. TAMS-AC has successfully delivered culturally appropriate primary healthcare services for First Nations People for close to 30 years. In addition to primary healthcare and day to day health services provided by a team of onsite doctors and nurses, TAMS-AC coordinates access to a range of visiting specialist services and community programs.

Tasks & responsibilities

• Provide outreach to Aboriginal and Torres Strait Islander communities in a culturally sensitive manner, breaking down barriers to accessing the NDIS and developing trust and rapport.
• Support potential participants and their representatives to attend appointments to get appropriate assessments and other evidence for NDIS requests.
• Supporting potential participants  with respect to submitting forms
• Linking Aboriginal and Torres Strait Islander people who are not eligible for NDIS to community mainstream supports.
• To support smooth access to NDIS.
• To identify service gaps, barriers and strengths.
• Follow direction of the Senior Management Team and organisation.
• Commit to achieving the Core Values of the company.
